Designated Physicians: Responsibilities and Required Forms for Office Surgery Facilities

From initial registration to reporting adverse incidents, DPs are responsible for managing all activities. Below is a guide to the key forms and their uses that every designated physician should be familiar with.
Office Surgery Registration Application
This application is essential for registering a new Office Surgery facility with the Florida Board of Medicine. It’s also used to update existing registrations, including staff changes and credential updates. Keeping this information current is a core responsibility of the DP.
Risk Management Records
All Office Surgery facilities must maintain a Policy and Procedure Manual that includes documentation of the facility’s risk management program. This manual should be reviewed and updated annually. Additionally, DPs must document quarterly risk management meetings and file reports of any adverse incidents.

Adverse Incident Reporting Form

Facilities must provide patients with the name and address of the nearest hospital to which they would be transferred in an emergency. This document must also be stored in each patient’s records.
Staff Privileges and Transfer Agreements
Staff privileges and hospital transfer agreements often have expiration dates. DPs must implement a tracking system to ensure these agreements are renewed on time. Updated documents must be submitted to the Board of Medicine when applicable.
