Helpful Tip: National Practitioner Data Bank

The Florida Board of Medicine and the Florida Board of Osteopathic Medicine requires that an applicant for licensure submit a National Practitioner Data Bank (NPDB) Report to their appropriate Board.  The NPDB houses reports from various reporting entities regarding medical malpractice and adverse actions related to health care practitioners.

Recently, the NPDB discovered and investigated an issue regarding forged reports being provided to licensing boards.  As a result, the Florida Boards now require a digitally certified self-query report be submitted when applying for a license.  For more information on digitally certified self-query reports, visit the NPDB at this link: https://www.npdb.hrsa.gov/faqs/sq22.jsp.
